Does exactly what it promises - and does ist good.
I needed a small patch to make it work. I use a database table name prefix, and the extension does not consider this by default.
In AddressController.php, CategoryController.php and CustomerController.php at line 36 or so, replace
$model->setData("sort_order",$db->fetchOne("select sort_order from eav_entity_attribute where attribute_id=$id"));
$tableName = Mage::getSingleton('core/resource')->getTableName('eav_entity_attribute');
$model->setData("sort_order",$db->fetchOne("select sort_order from $tableName where attribute_id=$id"));
to make it work with table prefixes.

Should be Core
These days this is the first extension we put in a new Community build.
It's been invaluable. No more fiddling around with setup scripts and it even allows clients to safely add their own attributes.
Even more power - you can remove attributes that are no longer needed.
Personally I think this should be built in!

Works on 1.9
This extension works for me on 1.9, I am using the customized rwd theme.
Please note that I just use the "Customer Address Attributes"
I created a new Attribute with the Attribute code <b>"doorcode"</b>
Then I opened this file in my theme folder:
Look where you want to put the field, in my case underneath Zip/Postal code, then add a list entry.

<input type="text" name="doorcode" id="doorcode" value="<?php echo $this->escapeHtml($this->getAddress()->getDoorcode()) ?>" />
Take a good look at this fragment, this calls the attribute:
In case your Attribute Code is "haircolor"
Then the code would be:
To display the Attribute in the Invoice, navigate to
System > Configuration > Customers > Customer Configuration. There expend address templates.
What I did was copy every instance of postcode and rename it doorcode.
Now Soft bounce your server and you have fully functioning address attributes.
