I have used the surcharge extension for over a year and I have to say that it is a fabulous extension. No gripes from me in terms of functionality. I did discover some bugs in the past year but Kristof was very responsive and was able to fix the problems pretty quickly. I highly recommend this module.
BUT I need to make one BIG, important disclaimer. I just got notified by my bank that it is against my user agreement with Visa and Mastercard to do a surcharge on purchases with those cards. No problems with Discover/AMEX, only Visa/Mastercard. We are about to get charged $5000 unless we stop using the surcharge for Visa/Mastercard purchases. Ouch.
I am in Texas, USA where it is against the agreement. Other US states may not have the same restriction... and I have no idea about other countries. But I just wanted to warn everyone so you can do your own due diligence. This link might be old, but it has some good info. http://answers.google.com/answers/threadview/id/723060.html. You will definitely want to contact your bank. And I wonder about Paypal?

I have followed all instructions after a manual install (on PE, do not have magento connect), and everything is working except I have no "stats" tab in the backend to view the live stats. I do get the email at the end of the day with the correct stats, but I do not see it in the backend. I might have to move some things around on the server to get it to show up properly, but I will let you know.

I was hoping this would work for me but unfortunately it broke one of my most critical extensions called dependent custom options. It basically broke dependency so that all of my custom options were expanded out and no longer dependent. Perhaps this isn't compatible with that extension. However, I also noticed that when I logged into the Admin none of the links worked. I couldn't go anywhere and I was stuck on the dashboard. The css wasn't gone like others have mentioned, and everything looked normal but I just couldn't do anything, none of the links worked, it was weird. I eventually had to log into the downloader to uninstall and as soon as I did everything went back to normal and started working again. I'm so bummed...
ps. I'm referring to version 2.0.5

Great extension! Installation was a breeze... just make sure your folder and file permissions are correct.
I have a FIOS connection and with that you can't tell any difference but I installed the Firefox Throttle extension for Firefox and set it to default at 56K connection speed. Before installing Speedster it took 90 seconds to load at 56k... a minute and a half. After installing Speedster, adding cache control headers, and of course clearing browser cache between tests, it loads in 22 seconds flat. Amazing!
On GTmetrix I was gettin a D's for both YSlow and Page Speed, now I get A's for both, not to mention page size and number of requests are way down.
I haven't fully tested the cart but so far it seems to work without issue.
Great work Fooman!

WOO HOO, the sagepay problem has finally been fixed. (Wish I'd spotted it had been updated sooner. I use to like that about Magento Connect, it would always list newly updated first)
Anyways, WOO, works on Magento 188.8.131.52 a treat now.
