With this extension you can create custom user surveys that integrate seamlessly with your Magento store. View customer feedback by looking at paginated survey responses, charts, and graphs. Not only that, you can give the user incentive to fill it out by granting them a coupon code upon completion.

To create a test survey, log in with the above credentials then click on the Customer Survey tab in the top navigation, far left side.
