This is brilliant! Really easy to use. Just click Refresh and it shows you a list of all the images not being used. You can see the images, so can check them before clicking delete.

This is a great extension.
Tried on 22.214.171.124 (Upgrading Today)
Many thanks to the developer!
15239MB (1.5GB) of Space Freed
It found over 126,000 images
Shows short image path on image hover
Shows actual images
Allows deletion of 1, all or any number of images from admin with a couple clicks.
CONS: None though you should always check what you're deleting before you delete it. It might pick up non-product images.
As with anything that does a big task in Magento (like deleting lots of products at once), it appears to hang in your browser until it's done finding the images. But that's not the extension, it's Magento CE. Be patient if you have a big site.
After installed, the extension is found in your top nav bar.
Logging out and back in doesn't appear to lose the found image list.

6500+ files eliminated and 1GB recovered. It can be done manually with a dump of the Gallery table compared against "du" command output (SQL is Fun!) but this extension takes the win!
