If a create a new simple product from a configurable product page, either with the Quick Product dialog or by copying the template from the configurable product, the simple product is created but is not associated with the configurable product. It does not appear in the list of associated products either in admin or on the front-end even though it has the required attributes to be included.

Creating a simple product from scratch with the required attributes does not work either.
