Actually, this error message is shown instead of

Notice: Undefined index: qty in /var/www/miamaggie/magento116/app/code/core/Mage/Adminhtml/controllers/Sales/Order/CreditmemoController.php on line 462

Looking at the code, there is a similar code at line 456 which might result in the same error in some other circumstances.

Note: the payment method which was used for the order in question does not allow partial capture, that's why (i guess) the quantities to refund are displayed with non-editable plaintext zeroes at the credit memo creation page? :))
