Adding custom module with form to admin area?

Hi,

Does anyone know how to add a custom admin module for the admin area? The wiki: http://www.magentocommerce.com/wiki/how_to_create_an_admin_form_module no longer applies to 1.0 since there is NO Mage directory under app/code/local.

Can someone show how to do it with 1.0. I would also be interested in the frontend version of this as well.
