certain category / modules using wrong template

I’m at a loss to figure out why this is happening or locate where the problem is.

I have two categories,
/Root Category/Series/name-a, url key = “name-a”
/Root Category/Series/name-b, url key = “name-b”

I’ve compared the settings for both categories side by side and they’re identical.
If you try to goto one (http://storeurl.com/series/name-a.html), it loads fine.
If you try to goto the other (http://storeurl.com/series/name-b.html), all I get is a blank page.
The only indication that I can see that something went wrong is inside the /var/log/system.log file, that shows the following line

2013-07-02T18:38:18+00:00 CRIT (2): Not valid template file:frontend/base/default/template/page/2columns.phtml

now I understand that their’s a heirachy that it’s following, and that page doesn’t actually exist under /frontend/base/default, however it does exist within my package/theme’s directory, which is what name-a uses just fine. Only when trying to use name-b does it fail out.

I’ve manually emptied /var/session & /var/cache, as well as regenerating all indexes multiple times. I still cannot figure out why one works and the other doesn’t.

Can anyone shed light on where else to look where there might be lingering code or something interfering with it?
