404 errors after Catalog Rewrite refresh

Hi, I am getting 404 errors from all category links after having refreshed Catalog Rewrites in the admin.

Homepage works fine and products work fine too:

http://www.concretehermit.com/magtest/index.php/monsterism-pets-and-owners.html
http://www.concretehermit.com/magtest/index.php/toys/monsterism-pets-and-owners.html (addition of category ‘toys’)

but
http://www.concretehermit.com/magtest/index.php/toys/

(and all other categories display 404)

I have followed many threads and tried many solutions, but can’t get it fixed!
Refreshed/ disabled cache.
refreshed catalog rewrites - which I think caused the problem.
replaced .htaccess file with default sample
checked base url is set correctly
renamed deleted and added new categories
turned off seo rewrites

prior to the initial press of refresh Catalog Rewrites in the admin everything was fine and I was near to going live!!

all other links are working, just can’t get my categories back.

SOME PROGRESS -

I deleted all my existing categories in admin and then re-entered one category and added one product to that category. Refreshed cache and rewrites.

Now when I click on the category from the home page it displays! BUT the url looks a little odd:

http://www.concretehermit.com/magtest/clothing/ DOESN’T WORK
BUT
http://www.concretehermit.com/magtest/clothing/-1 DOES.

Any Idea what the -1 is referring to, why it is added and what the significance to my broken categories is?

I upgraded to 1.2.03 but still getting issues. I’ve made a back up and have decided to to a complete reinstall… Db and everything.
