So sorry for your recent troubles. Fortunately, we’ve seem them all, and in this situation, the best steps you can take is to take a full backup of your files and database.

Next, load up a second, fresh store of version 1.4.1.0 with the same database prefix into a subfolder, for example, domain.com/clean

Download and insert the Magento database repair tool located here:

Into your document root.

Run the repair tool and fill out a brief bit of information, and it should automatically clean things up and fix any problems.

Your magento installation was not upgraded properly.
sales_flat_order table should exist definitely.

If you have backup of non upgraded installation, it\’s very cool.
If no - use repair tool, as SimpleHelix.com said.
